Healing Complex Trauma with EMDR: Understanding the Process, Side Effects, and Window of Tolerance

Let’s set a familiar scene. You have a major project due, or perhaps your home desperately needs cleaning. You know exactly what needs to be done. You know how to do it. You even genuinely want to get it done.

Yet, you find yourself organizing your spice rack alphabetically, falling down a Wikipedia rabbit hole, or simply staring at the wall as a low-grade panic sets in. You have 47 browser tabs open on your computer, and another 47 running concurrently in your brain.

If this experience feels familiar to you or your child, welcome to the club. You're likely experiencing challenges in your executive functioning, a core feature of Attention-Deficit/Hyperactivity Disorder (ADHD).

At VMA Psych, we frequently work with individuals who describe this exact pattern.

ADHD can feel like having the capacity to perform without consistent access to the systems that support follow-through. The good news is that these skills can be developed. One of the most effective, evidence-based approaches is ADHD Coaching, also referred to as Executive Functioning Coaching.

It’s Not a Knowledge Gap, It’s a Performance Gap

Historically, ADHD was often misunderstood as a simple issue of hyperactivity or inattention. Contemporary research has clarified that ADHD is a neurodevelopmental condition affecting self-regulation and executive functioning, primarily associated with differences in the prefrontal cortex.

Neurobiologically, ADHD is linked to differences in dopamine and norepinephrine regulation, which influence motivation, attention, and reward processing. As a result, tasks that are repetitive, delayed in reward, or lacking stimulation can feel disproportionately difficult to initiate and sustain.

Leading researcher Russell A. Barkley conceptualizes ADHD as a performance disorder, not a knowledge deficit. In other words, individuals with ADHD often know what to do, but struggle to consistently translate that knowledge into action.

What Are Executive Functions?

Executive functions are the cognitive processes that enable goal-directed behaviour. They allow us to plan, initiate, organize, and regulate our actions over time. You can think of them like the CEO of the brain.

These skills include:

  • Task Initiation: Beginning tasks without excessive delay

  • Working Memory: Holding and manipulating information during complex activities

  • Time Management: Accurately estimating and allocating time

  • Emotional Regulation: Managing frustration and maintaining persistence

  • Planning and Organization: Breaking goals into structured, actionable steps

When these systems are underdeveloped or dysregulated, even highly capable individuals can experience significant challenges in daily functioning.

Why Traditional Advice Often Falls Short

Common advice, such as “just use a planner” or “try harder,” assumes that the difficulty lies in effort or knowledge. However, for individuals with ADHD, the challenge is neurological—not motivational.

This is why traditional strategies often fail. Without addressing the underlying executive functioning differences, these approaches can lead to frustration, inconsistency, and self-criticism.

Enter ADHD Coaching

ADHD Coaching is a structured, goal-oriented intervention designed to improve executive functioning in real-world contexts. Unlike traditional psychotherapy, which may focus on insight and emotional processing, coaching emphasizes practical skill-building, systems development, and accountability.

Research supports a multimodal approach to ADHD, where behavioural interventions such as coaching are combined with other supports (e.g., therapy or medication, when appropriate) to improve outcomes.

Here is how a trained ADHD Coach changes the game:

  1. Building Brain-Compatible Systems

ADHD brains respond strongly to interest, novelty, urgency, and reward. Coaching focuses on designing systems that align with these drivers. This may include breaking tasks into shorter intervals, introducing external structure, or using techniques such as body doubling to sustain attention.

  1. Externalizing Executive Functions

Coaching provides structured support, often called scaffolding, to compensate for gaps in executive functioning. Complex tasks are broken into manageable steps, reducing overwhelm and increasing follow-through. Over time, these skills become internalized.

  1. Consistent Accountability

Follow-through is one of the most common challenges in ADHD. Coaching introduces regular, non-judgmental accountability, helping individuals maintain momentum and build consistency over time.

Who Benefits from ADHD Coaching?

ADHD Coaching can be beneficial across the lifespan:

For Children and Teens:

  • Developing academic skills and organization

  • Improving emotional regulation

  • Building self-advocacy and independence

For Adults:

  • Enhancing workplace productivity

  • Managing time and responsibilities

  • Reducing chronic procrastination and overwhelm

  • Addressing the long-term impact of undiagnosed or unmanaged ADHD
